Attach Forms to Service and Rental Bookings In-App
TL;DR
Users can now attach and complete forms directly inside the appointment modal when creating service and rental bookings.

TL;DR
You can now attach and fill out forms directly in the appointment modal when creating service and rental bookings. No immediate configuration changes are required unless your team creates these bookings manually.
Who this affects
HighLevel has not confirmed specific plan limits or user tiers for this feature. Anyone booking service or rental appointments manually inside the platform can use it. Agencies that do not offer service or rental bookings can safely ignore it.
What changed
- A new "Booking form" section with an "Attach form" button appears inside the in-app appointment modal for service and rental bookings.
- Configured forms are preselected by default, with an option to switch forms using a dropdown.
- Contact details—specifically first name, last name, email, and phone—automatically prefill from the selected contact.
- Elements for CAPTCHA, product selling, and payments are automatically hidden when filling the form within the modal.
- Form attachment is restricted to initial booking creation; you cannot attach or edit forms after the appointment is created.
- Completed forms can be reviewed directly in the appointment details.
- Plan gating, replaces/breaks behavior, and full rollout status have not been confirmed by HighLevel.
Where to find it
HighLevel has not published the exact navigation path for this release. It lives inside the in-app appointment modal displayed when manually scheduling a service or rental booking.
How agencies will use this
When a staff member takes an inbound call from an existing contact to schedule a rental or service, they open the appointment modal. After selecting the contact, the system prefills their first name, last name, phone, and email into the attached booking form. The staff member switches forms using the dropdown if needed, completes any intake questions, and submits the booking. Once saved, the team can review the submitted intake information later directly inside the appointment details.
Our take
This is a minor, quality-of-life update. It saves staff from manually copying contact data or handling intake questions across separate screens during manual scheduling. The hard restriction that prevents adding or editing forms after the initial booking is created is a clear limitation to keep in mind.
